Special Events Vendor Agreement

The above business agrees to provide food/beverage services for the following event/dates and pay the permit fees per event.

Your application will be reviewed shortly and staff will contact you upon approval. At that time, payment will be collected. Please submit payment via check, payable to "City of Ketchum," in person at City Hall or mailed to the address below. Credit cards cannot be accepted.
